This is an early vintage Great Neck New York Brilhart Tonalin Alto mouthpiece.  The piece has been professionally refaced  for its original 3 facing to a new ".070.  Whoever did the facing work did a fantastic job.  The mouthpiece still has its original box.  The Great Neck NY models are very highly regarded as great alto pieces, and are beoming harder and harder to find.
